Recipe for classic Carbro bleach

Hello!

I was searching thru APUG and internet about the exact recipe for Carbro bleach mixture. Mostly i have found out the chemicals needed, but the mixing ratio between the chemicals is still an issue. Can anybody help me with this? Many thanks to all in advance!
